New Client,  Welcome!
To give you the best possible coaching start, prior to your first coaching session,
your coach needs to receive from you the following:

1. New-Client Contact Information online form includes Terms of Service (4-8 mins).
   To access the above form, request the "Generic UserId  and Generic Password" 
   by e-mail from Joseph, Kande, Naomi, Susan or Veronica.
    
2. Quality of Life Check-up online form (7-15 mins). You can do this step before you get
    the generic password. (Free Adobe Reader** set by you to "open in browser" is used.)
    
3. Payment for your monthly coaching package* (see rates)

* Payment arrangements for corporate coaching are made directly with accounting departments.
